Item Donation Form
If you are planning to donate items, please complete the following form once for each item you're going to donate.
The item donation form must be submitted by September 24. Drop donated items off at the Chamber office (1314 W. Moore Ave.) no later than Friday, October 4 at 4 p.m. Online bidding for the silent and premier auction items will open on Monday, October 14.
