If you are planning to donate items, please complete the following form once for each item you're planning to donate.

To be included in the electronic auction catalog, the item donation must be submitted by September 28. Drop donated items off at the Chamber office (1314 W. Moore Ave.) no later than Friday, October 2 at 4 p.m.
